Each child needs there own registration form completed.

*Current Physical
*Insurance Info
*Payments in person
Athlete Registration fee:
Accounts payable to AMATEUR ADVOCATE ASSOCIATION INC.
(Memo: PJF Track)
Payments are directly applied toward association fees per athlete participating in any CCTYL and PJF spring track meets.
Only cash, check or money order at this time.
